Comedonal variant of chronic cutaneous lupus erythematosus on the nose

A thirty-year-old patient presented with an erythematous papule on the left nostril evolving for ten months. clinical examination revealed infiltrated, erythematous, well-limited plaque a raised border, covered multiple open and closed comedones. On dermoscopy, there was background some fine telangiectasias horny plugs at follicular orifices. skin biopsy performed, revealing orthokeratotic hyperkeratosis sinking into orifices dilated by sebum clumps basal vacuolation associated subepidermal periadnexal/perivascular lymphocyte band infiltrate. Direct immunofluorescence staining immunoglobulin M positive. The diagnosis of lupus comedones retained, put topical tacrolimus 0.1% twice day. systemic damage assessment negative. Our case highlighted, importance recognizing this rare variant cutaneous lupus, confused acne vulgaris, hence delayed diagnosis, which may also be early sign concomitant involvement.
